I would suspect this is down to budgets within labels which are probably quite small compared to the budgets of the promo heyday back in the '80s. Physical promo formats are produced in ever smaller numbers given the demise of record stores needing them, coupled with radio using digital tracks to plug new music. Historically many US promotional pressings did not come with front inserts, often just a custom back insert: I agree they look odd but that's how the label chose to release them - again probably budget driven (it was fairly typical of early Warner/Atlantic promos).
